Schedule an Appointment
To schedule an appointment at one of our three
convenient locations please call: (330) 729-9214 for Boardman and (330) 779-6851 for Austintown & Warren
- You will be asked to give your name, address,
telephone number and name of your insurance provider.
- Please let the receptionist know if you have a
preference with respect to a location or one of our
five physicians.
- Please check with your insurance provider to see if
you are required to have a referral from you primary
care physician prior to seeing our physicians.
- At your initial visit please bring with you a current
list of any medications that you may be taking and a
current copy of your insurance card.
Please come to all visits prepared to give a urine
specimen.
